Have dinner with her after developing interaction at a certain stage of the relationship.
Rescue her from the Collectors after the crew of the Normandy is kidnapped (i.e. rescue them as soon as possible after the kidnapping).

How to make sure Kelly survived the attack on the Citadel
You need to talk to Kelly Chambers as often as possible, checking on her before and after each major story mission. Soon she will reveal that she is actually spying on you for the Cerberus and that she has been sending reports about you to the Illusive Man.
It is very important that you forgive her after this is figured out, otherwise she will kill herself out of guilt for her actions.
You also need to tell her to change her personality after you have forgiven her; otherwise, she will not survive the attack on the Cerberus Citadel that occurs during the game’s main story missions.
As long as all of these requirements are met, Kelly will live to the end of the game, changing her name to Felicia Hannigan. She will then put you in contact with others.
